Because insurance coverage and provider networks can change, we encourage patients to contact their insurance carrier before their appointment to verify that our physicians participate in their plan and to understand their individual benefits. We can still see you if your plan offers out-of-network benefits.
Referrals & Authorizations
Some insurance plans require a referral from your primary care physician before your visit. If your plan requires a referral or prior authorization, please ensure it has been obtained before your appointment to help avoid delays in care.

Patient Financial Responsibility
Patients are responsible for any applicable copayments, deductibles, coinsurance, and services not covered by their insurance plan. Payment is expected at the time of your visit unless other arrangements have been made. If you have questions regarding your insurance coverage or billing, please contact our office. Our staff is happy to assist you whenever possible.
